This Consumer Health Data Privacy Policy explains how Boost Claims ("BoostClaims," "we," "us") collects, uses, and shares consumer health data, and the rights available to residents of states with consumer health data laws, including Washington, Nevada, and Connecticut. It supplements our general Privacy Policy.

Consumer Health Data We Collect

• Descriptions of injuries you tell us about, including the type and severity of injury;
• Information about medical treatment you received or expect to receive;
• Incident details that may reveal information about your health or medical condition.

We collect this information from one source: you, when you choose to answer our survey questions. We do not collect health data from third parties, and we do not use inferences, precise geolocation, or biometric data to derive health information.

Why We Collect and Use It

• To evaluate and route your request for a claim review;
• To connect you with participating law firms and legal service providers relevant to your potential claim;
• To document your consent and comply with legal obligations.

How We Share and Sell Consumer Health Data

With your consent, we share consumer health data with the following categories of recipients, and in some cases sell it to them together with your contact information:

• Participating law firms and attorneys who purchase or receive leads from us;
• Marketing companies and lead buyers who connect consumers with law firms;
• Service providers that host or process data on our behalf.

Where state law requires it (including for Washington and Nevada residents), we will not sell your consumer health data unless you first sign a separate, specific authorization describing the sale. You are not required to sign an authorization to use this website, you may revoke an authorization at any time, and we will provide you a copy of any authorization you sign.

Your Consent

We collect and share consumer health data only after you affirmatively submit it through our survey following the consent disclosures presented at submission. We do not collect consumer health data for purposes beyond those described here, and we do not use it for advertising unrelated to your inquiry.

Your Rights

• Access — confirm whether we collect, share, or sell your consumer health data and receive a list of recipients;
• Withdraw consent — stop future collection, sharing, or sale;
• Delete — request deletion of your consumer health data, including from our backups and, where required, notification to recipients;
• Non-discrimination — we will not penalize you for exercising these rights.

To exercise any right, email jimmy@boostclaims.com from the address you used with us, or include enough information for us to verify your identity. We will respond within the time your state's law requires. If we deny your request, you may appeal by replying to our decision; if your appeal is denied, you may contact your state Attorney General (for Washington residents, at atg.wa.gov).
